Instructions
Download the printout and cut out the card along the outside dotted line. Then fold it along the inside dotted line. Secure the front and back with tape around the edges or insert the card into a protective plastic sleeve.

• For Healthcare Providers Print and keep a copy of the card as a quick reference on gender-affirming care. 

• For Patients/Clients Print and keep the card to share with your healthcare providers, request that your providers include the card in your health record, and/or print copies of the card for your providers to keep.
